In Los Angeles, Breakthrough Infections Are Now 30% Of All New Covid Cases Amid Delta Surge

In March 2021, when Americans first began hearing about the Delta variant of Covid-19, breakthrough infections among fully-vaccinated Angelenos accounted for just 2% of that month’s case total. By June, when Delta accounted for 50% of all variants in Los Angeles, breakthrough infections among fully-vaccinated residents had risen to 20% of all identified cases. In late July, county officials reported that the Delta variant had overwhelmed all others, accounting for more than 90% of all positive tests analyzed for variants. As a result, the ratio of breakthrough cases increased to 30%, local health officials announced today.

Real-world data from Israel has revealed that Pfizer-BioNTech’s COVID-19 vaccine is 94 percent effective in preventing asymptomatic cases, the companies said Thursday.

Using data from Israel’s vaccination campaign between Jan. 17 and March 16, the vaccine was also found to prevent 97 percent of symptomatic disease, severe disease and death, the drugmakers said.

According to an agreement signed between the government and these hospitals, they were supposed to receive a 630-million-shekel budget between January and June 2021. Of this money, 300 million shekels were for their routine operations and 330 million shekels were for coronavirus related work. The agreement was also supposed to provide another 55 million shekels a month between June and December. However, the government has only provided these hospitals with a total of 300 million shekels.

Prefatory note: In this post, I take the noun „vaccine“ as the basic word under discussion, but also consider other cognate terms („vaccinate“, „vaccination“).

Here‘s a standard dictionary entry for „vaccine“:

For the purpose of this surveillance, a vaccine breakthrough infection is defined as the detection of SARS-CoV-2 RNA or antigen in a respiratory specimen collected from a person ≥14 days after they have completed all recommended doses of a U.S. Food and Drug Administration (FDA)-authorized COVID-19 vaccine.

Identifying and investigating hospitalized or fatal vaccine breakthrough cases

CDC monitors reported hospitalized or fatal vaccine breakthrough cases for clustering by patient demographics, geographic location, time since vaccination, vaccine type, and SARS-CoV-2 lineage. Reported data include hospitalized or fatal breakthrough cases due to any cause, including causes not related to COVID-19.

The CDC‘s recent shift in monitoring breakthrough COVID-19 infections after vaccination hasn‘t sat well with everyone, as some scientists are concerned it could miss the full picture of what is happening with these breakthrough cases.

As of May 1, the agency went from monitoring all breakthrough infections after COVID-19 vaccination to focusing on only hospitalized or fatal cases.

Definition wahrscheinlicher Impfdurchbruch:

Ein wahrscheinlicher Impfdurchbruch ist definiert als SARS-CoV-2-Infektion (mit klinischer Symptomatik), die bei einer vollständig geimpften Person mittels PCR oder Erregerisolierung diagnostiziert wurde. Ein vollständiger Impfschutz wird angenommen, wenn nach einer abgeschlossenen Impfserie (2 Dosen Moderna-, BioNTech- oder AstraZeneca-Vakzine bzw. 1 Dosis Janssen-Vakzine) mindestens zwei Wochen vergangen sind.

Die Impfkampagne in Deutschland hat Ende Dezember 2020 begonnen, im Folgenden sind Informationen zu Fällen und Impfdurchbrüchen ab dem 01.02.2021 dargestellt. Dies ist durch die Definition eines Impfdurchbruchs begründet (mindestens zwei Wochen nach zweiter Impfstoffdosis sowie Impfintervall von mindestens drei Wochen zwischen erster und zweiter Impfstoffdosis: frühestmöglicher Impfdurchbruch ab 01.02.2021).Insgesamt 8.715* Impfdurchbrüche wurden seit dem 01.02.2021 anhand der nach IfSG übermittelten Meldedaten identifiziert, davon 6.586 nach einer abgeschlossenen Impfserie mit Comirnaty (BioNTech/Pfizer), 297 mit Spikevax (Moderna), 493 mit Vaxzevria (AstraZeneca) und 934 mit COVID-19 Vaccine Janssen. Bei weiteren 405 Impfdurchbrüchen erfolgte anhand der vorliegenden Angaben keine Zuordnung zu den o.g. Impfstoffen.

Denn wer die Impfung propagiert, hat kein Interesse an einer wachsenden Zahl von Impfdurchbrüchen. Die würde der laufenden Impfkampagne zuwider laufen. Und die oben genannte Definition, festgelegt vom Robert-Koch-Institut, ist dieser Mission ziemlich dienlich.

Denn ausdrücklich nicht als Impfdurchbruch gilt es, wenn eine vollständig geimpfte Person zwar beim PCR-Test ein positives Resultat aufweist, aber keine Symptome hat, also weder hustet noch sonst kränkelt. Wer mit dem Impfstoff im Körper positiv getestet ist, sich aber wohl fühlt, der hat also keinen Impfdurchbruch.

In einem Beitrag auf Facebook behauptet ein Nutzer, bei einem Geimpften würde ein positiver PCR-Test vom Robert-Koch-Institut (RKI) nur dann als Impfdurchbruch gezählt, wenn die Person eine „klinische Symptomatik“ habe.

Im Situationsbericht des RKI vom 5. August heißt es dazu auf Seite 17: „Ein wahrscheinlicher Impfdurchbruch ist definiert als SARS-CoV-2-Infektion (mit klinischer Symptomatik), die bei einer vollständig geimpften Person mittels PCR oder Erregerisolierung diagnostiziert wurde.“
